Adaptive Designs for Dose Escalation Studies
In Phase I dose escalation studies the 3+3 design is a commonly used method. We suggest a modification of this method to include a parametric model and the concepts of optimal design theory, with the aim of improving this method. The performance of the original and the new approach are investigated by simulations.
متن کاملAddressing the barriers to implementing model based designs in dose escalation studies
Dose escalation studies are undertaken to determine the recommended dose for later phase trials, often the maximum tolerated dose that can be administered beyond which the rate of toxicity events becomes too high. The advantages of model-based designs over rule based approaches (such as the 3+3 design) have been established for many years. While further innovations in design are being developed...
متن کاملCommentary on 'Designs for dose-escalation trials with quantitative responses'.
Bailey (Statist. Medi 2009; this issue) puts the focus on efficiency in dose-finding studies and shows that some quite simple modifications to much current experimentation can lead to significant efficiency gains. Efficiency has not been the main focus of interest in this field, in particular when dealing with situations requiring other than healthy volunteers. Dr Bailey's results are important...
متن کاملThe construction of optimal designs for dose-escalation studies
Methods for the construction of A-,MV -,Dand E-optimal designs for dose-escalation studies are presented. Algebraic results proved elusive and explicit expressions for the requisite optimal designs are only given for a restricted class of traditional designs. Recourse to numerical procedures and heuristics is therefore made. Complete enumeration of all possible designs is discussed but is, as e...
متن کاملEfficiency of New Dose Escalation Designs in Dose-Finding Phase I Trials of Molecularly Targeted Agents
BACKGROUND Statistical simulations have consistently demonstrated that new dose-escalation designs such as accelerated titration design (ATD) and continual reassessment method (CRM)-type designs outperform the standard "3+3" design in phase I cancer clinical trials. METHODS We evaluated the actual efficiency of different dose escalation methods employed in first-in-human phase I clinical tria...
